The access center staff is available to answer any questions you may have regarding any of our services. Upon contacting our access center, you will be assigned an Admissions Case Manager who will assist you in determining what services best suits your needs. For educational and/or residential services, the case manager will set up a pre-placement interview and tour. Most interviews occur at one of Grafton’s sites in order for applicants, family members, and agency representatives to have the opportunity to tour the site and meet prospective team members. Should a visit to one of the sites not be possible, the interview can be completed at the applicant’s current location. The interview is an open exchange of information to get to know the applicant better and for the applicant, families, and agencies to obtain information about Grafton as well. The pre-placement interview assists in determining the appropriateness of services, specific outcomes being sought for and by the individual, related services needed and the approximate length of stay required to accomplish the desired outcomes. We have found that having clear expectations around these issues helps with individual treatment and communication among all parties.

Providing the following information will assist in expediting the decision making process:

  • application (the Admissions Case Manager is available for assistance if needed)
  • dental exam
  • discharge summaries from previous placements
  • educational, speech, occupational and physical therapy evaluations (if appropriate)
  • individual family service plan (if applicable)
  • insurance information
  • medical history, current medications, physical & immunization records
  • psychiatric report, if applicable
  • recent psychological evaluation
  • social history including both family and developmental history
  • school history, including a current IEP – Individual Education Plan – for clients under 21 years of age
  • school transcript, credits earned, if appropriate
  • treatment history, if appropriate
