An initial plan of care is developed within 24 hours of enrollment to the program. Once an enrollment date has been established, time-limited step-down plans are developed for each student with funding agencies, parents/guardians and the student. Grafton’s internal utilization management process begins to monitor progress of the plan. Within a month of enrollment, if the student is “off-plan,” specific measures are taken, including consultation with funding agencies and families, as appropriate. Our goal is always to provide the necessary treatment in order to return the student to his or her home community or to a less restrictive level of care as rapidly as possible.
Residential ProgramOnce clients are enrolled in residential programs, they are provided the opportunity to make weekly phone calls to their families/guardians. Families/guardians are encouraged to visit and see how the family member is doing, get to know the staff and participate in designated activities. When appropriate, clients will also have opportunities to go home on weekends or holidays. It is our goal for all residential students to return to their home community, thus continued contact with the home environment and meaningful interaction with people in that community are critical.
Our staff will be in close contact with families/guardians and agencies. We want to communicate with you about how your child is progressing and in what activities he/she is participating. We also need to maintain a dialogue with you about your expectations for your child. Grafton recognizes that we work with clients for a short time in their lives, and that even during that time, we represent only one of the individual’s important relationships. We hope our influence is significant, but our time with each client is indeed brief. The more productive our work with families and agencies, the more likely the client will have a successful stay at Grafton and a positive transition back to the home community.
Group HomesFor people who choose to live all or part of their adult lives with Grafton support, our goal is to help maintain and strengthen previous ties to family and friends while supporting the development of new relationships. Agency personnel provide the essential direction, guidance and friendship needed to navigate the service delivery system. Grafton respects the unique nature of these relationships and strives to maintain consistent and productive communications with all who play major roles in the lives of the individuals we serve.
