Comprehensive Assessments

Getting a full understanding of our clients’ conditions is at the heart of what we do. A full array of assessments are conducted by Grafton’s multi-disciplinary team and consultants. Our front end evaluation and diagnostic process occurs during the several weeks prior to the client’s admission and for approximately 30 days after the admission. A full array of assessments will be conducted using our Grafton multidisciplinary team and community-based consultants. The evaluations and assessments will include medical, psychiatric, functional analysis, educational, and psychological as appropriate. Depending on the needs of the client, evaluations and assessments may expand to include speech, occupational, vocational, and neurological, and others as recommended by the treatment team. The purpose of this process is to provide individualized quality of care while at Grafton and to ensure client centered transitional and discharge planning.
