An initial plan of care is completed within 24 hours of admission. A case formulation, encompassing historical information and information gathered during the interview is then developed that includes the following components:
A Comprehensive Plan of Care (CIPOC) is completed by the 14th day of admission. This CIPOC drives all interventions in all settings while the client is at Grafton. A holistic approach to treatment is emphasized with the following components driving all plans:
The multidisciplinary team reviews each client’s plan on a monthly basis to ensure that the client is making progress and is on track with his/her goals. Interventions and goals may be adjusted if the client is not on track with meeting goals and objectives. |
