Independent Living Program for Teens Launches in Leesburg

Leesburg, Virginia, August 4, 2014 –Grafton Integrated Health Network (Grafton) is pleased to announce the launch of our Independent Living Program for Loudoun County youth (in the custody of the Department of Social Services).  The program at Grafton’s Leesburg campus is designed to help provide individuals (ages 15-21) with services and skills to promote advocacy, leadership and independence.

The program is designed to help youth develop functional autonomy, positive decision making skills, and a healthy lifestyle within the context of natural community supports.

The Independent Living Program is a collaborative partnership with Loudoun County stakeholders and Grafton is committed to help young adults within the program utilize the resources available within their local community to experience a positive transition to adulthood.  Services are provided at Grafton’s Leesburg campus where young adults receive many recreational, social and vocational opportunities.

24 hour room and board with onsite supervision and access to the following:

  • Independent living assessments will include areas such as money management, food preparation, hygiene, social, health and sexuality, transportation, education, career development and exploration, emergency and safety, community resources and housing.
  • Integrated and coordinated care between family, placing agency, community resource supports, other service providers and other designated key stakeholders provided by Grafton’s Program Manager.
  • Career exploration and assistance with GED
  • Money management, housing, transportation

 

Denaya, an individual scheduled to receive independent living services shared, “This program is going to give me a fresh start.  It is helpful to have people in your corner and I will now be able to focus on my goal—becoming a Nurse.”

The grand opening for the Independent Living Program is August 4, 2014.
